When NOT to Use AI — Regulated Healthcare Guardrails
There are three clear situations where we should not use AI in a regulated healthcare enterprise — or at least not use it autonomously.
First, clinical or operational decisions where error directly impacts patient safety.
If the consequence of being wrong is patient harm, AI should support humans, not replace them. Human accountability must remain clear.
Second, decisions that require explainability for regulatory, legal, or compliance reasons.
If we cannot explain how the model reached a decision — for example in care authorization, safety, or regulated reporting — then AI cannot be the final decision-maker.
Third, areas where data quality, bias, or governance are not yet mature.
Using AI on unstable or poorly governed data creates false confidence and systemic risk. In those cases, improving data and controls comes before deploying AI.
So the principle I follow is simple: AI should augment judgment where risk is high, and automate where risk is controlled and measurable.
